One signed waiver needed per each participant. No need to sign a new waiver for a different level - the first one you sign is valid for all classes.

1. You agree to our code of conduct policy.

2. Tickets are non-refundable, however are transferable to another name.

3. Always wear safety glasses when cutting glass and using equipment. Keep hair tied back.

4. Do not wear articles of clothing or jewelry that hang off the body.

5. All clothing must be 100% natural fibers. Try hard not to wear synthetics.

6. Do not wear open toed or open heeled footwear (i.e., flip-flops, sandals or clogs) in the glass studio. Wear long pants during class.

7. Do not use any equipment without an instructor present.

8. NOTE TO PREGNANT PEOPLE: Weekly stained glass classes (not 1-day workshops) are risky in terms of lead exposure when restoring or working with OLD lead which is oxidized (since oxidize lead can aerosolize into particles and be inhaled). Because of this, they are risky for pregnant people and are not recommended for people breast feeding. If you are taking a 1-day workshop and are currently pregnant and/or breastfeeding, there is no risk of oxidized lead, only risk is getting lead on your hands and then ingesting in addition to the risks associated with inhaling Flux fumes. The studio is equipped with HEPA fume extractors to remove toxic flux fumes from the air and you are required to wear nitril gloves. A fume respirator will be at your disposal. If you still do not feel comfortable soldering, the teacher will solder your piece while you can sit on the couch in a separate room outside of the studio.

9. In connection with my involvement in Angie OKO glass art classes at OKO Designs Studio (420 Sutter Street, San Francisco CA 94108) or the Winchester Mystery House (525 S Winchester Blvd, San Jose, CA 95128), I hereby release OKO Designs, Angie OKO, and The Winchester Mystery House, its agents, representatives, successors, or assignees, including OKO Designs management and employees, from all liabilities, actions, claims, damages, demands, costs, and expenses which I am now or in the future have against them, arising out of or in any way connected with my participation in art classes, including enroute to or from the program or its related events.

10. I understand that the waiver includes, but is not limited to, all injuries to me and or loss of injuries to any personal property. I also understand that this waiver includes, but is not limited to, any claims that are based on my alleged negligence or any other action or inaction of any of the above parties.

11. By checking here, I acknowledge I have read and understood the safety issues and waiver that have been outlined, and voluntarily agree to be legally bound by its terms.

12. I declare that I will watch the required stained glass safety video (not required for 1-day workshops)

13. I declare that I will not come to class if I test positive for COVID19 and will wear a mask if I test negative but have symptoms like runny/stuffy nose or cough. The studio provides and requires the use of N-95 masks for people who appear sick to protect others. If you are booked for an "N-95 masked workshop" and appear to be sick, the studio reserves the right to reject you for that date and reschedule you to a different date for when you are no longer sick.

14. I hereby acknowledge this release from liability for accidental injury or illness which I may incur as a result of participating in any stained glass activity in OKO Designs Studio (San Francisco, CA), The Winchester Mystery House (Sa Jose, CA), the Museum of Craft and Design (Dog Patch, SF) or at your place of residence/work for offsite workshops. I hereby assume all risks connected therewith and consent to participate in this program. I agree to disclose my physical limitations, disabilities, ailments, or impairments which may affect my ability to participate in this program.
